If you are a new patient you have multiple options for completing the new patient paperwork:
- In office completion: Please arrive 15 minutes prior to your scheduled appointment time.
- Patient Portal: Complete and update your information through PATIENT PORTAL.
- Home print: Download and complete CLIENT HISTORY FORM to bring with you to your appointment.

- Appointment can include treatments & procedures performed the same day including injectables, and laser treatments.
- New Patient Appointment: $150+tax
- The total cost of your consultation will be deducted from any subsequent cosmetic procedure you choose to undertake. Essentially, this makes your consultation cost-neutral, should you choose to proceed with a treatment. Please note, that this offer to offset the consultation fee against the cost of a treatment is only applicable for procedures booked within a 30-day window following your consultation.
- Due to Dr. Tan’s renowned expertise and the premium, personalized service provided, a fee for cosmetic consultation is mandatory. This fee ensures your exclusive access to Dr. Tan’s time, allowing for a thorough discussion of your aesthetic requirements and the creation of a tailored treatment plan.
